Sunnydale Supportive Care is a lead service provider of accommodation and assistance to people living with disabilities and mental health diagnoses under the age of 65yrs, enabling them to maintain independence and a life fulfilled. 

Funding provided by both State and NDIS allows us to provide accommodation and support services in the West of South Australia, and we welcome your enquiries at any time.
If you live with a disability, or care for someone that requires assistance, we may be able to offer you the services that will allow for greater safe independence such as respite care, transition support and advocacy as you require.

[ OUR MODEL OF SUPPORT ]

Sunnydale Supportive Care delivers a unique client centered model of support that involves both our clients and our staff working together as primary stakeholders.Our clients and partners include those directly impacted by disability, condition or illness, and/or their nominees, family members, supporters, guardians, friends, medical and allied health teams etc.
Our professional care staff are carefully selected and matched to our clients and their needs.
Every situation is obviously different and so our services must be carefully and individually tailored to meet each person’s needs.  We first take the time to understand each client, their needs and their goals and we then work towards developing a solution to match.
Once the model of care is in place, there is an ongoing process of liaison and co-ordination to ensure the client’s needs and goals are continually being met.

[ OUR CLIENT BASE ]

‘Disability’ spans a wide spectrum, and conditions include Physical, Intellectual, Psychosocial, Neurological, Spinal, Genetic, Degenerative, other Permanent Impairments and more, from mild to extremely complex, from 18 years of age and up.

[ S E R V I C E S ]

Sunnydale Supportive Care is an established supportive care accommodation service, which provides quality care and comfort for people in a warm and friendly environment.

Set on a spacious site with modern and fresh decor and recent extensive building upgrades, clients find themselves settled and 'at home' in an instant.

And that's before they even leave the accommodation - what a location! Only a short walk from Sunnydale is the charming and charismatic Semaphore Road, offering banks, post office, library and countless shops and restaurants. The bus stop to Port Adelaide and West Lakes is situated outside of Sunnydale and a short stroll along Cave Street brings you to the Semaphore foreshore.

Staff at Sunnydale are thoughtful and caring, offering help in many ways while ensuring residents are treated with respect at all times.  Overall, we strive to offer a high standard of care which is designed to add to our resident’s quality of life.

[ SERVICES PROVIDED ON A DAILY BASIS ]

  • Assistance with personal hygiene and grooming
  • Medication management and administration
  • Case management, including assistance making appointments and transport, and providing reminders where necessary. 
  • Weekly onsite local doctor visit, or if desired your own G.P. can visit you
  • Support to maintain and manage personal relationships
  • Assistance with Activities of Daily Living, including personal laundry, room cleaning and bed linen changing
  • Dietician approved meals according to specific dietary needs
  • Visiting services arranged regularly including hairdresser, podiatrist and Allied Health Service
  • Staff available 24/7, with passive overnight assistance


[ PROGRAMS TO ENRICH YOUR LIFE ]

Port Side Community Church visit our site each Thursday and provide a range of different day program’s which provide skill development, community integration and

Port Adelaide / Enfield council also visit the site and offer a wide range of activities and work, this service is provided by a generous team of volunteers their time.


[ ACCOMMODATION ]

Single and share rooms are available with rates according to the accommodation.

Our fees are set at a maximum rate of 79% of the full pension (i.e. pension plus full rent assistance). Fees include all meals, morning tea, afternoon tea, supper, personal services as per service plan.

Respite accommodation is available and costs 79% of maximum rate, Board and Care subsidy plus an extra 100%.

Upon entering Sunnydale, two weeks rent is payable in advance.
Residents may terminate a contract at any time provided that 14 days’ notice of termination in writing is provided to the proprietor.

  • Extensive CCTV covering all entries, exits and Common Room with 24 Hour Security Monitoring to foster a safe and secure environment.
  • Security Alarm System – Monitored 24 Hours
  • Panic/Duress Alarm & Intercom Service – for night time and after hours

  • Assistance with discretion and privacy so as to maintain the resident’s dignity
  • Access to any required services as set out in our clients ‘personal service plan’
  • Personal services carried out in a person centered approach, in accordance with our clients ‘personal service plan’
  • Upmost privacy when talking to professional health services, visitors, health case managers and other clients
  • Use of all the beautiful common areas within the grounds of Sunnydale
  • Respect and courtesy from our caring team of support staff
  • Right of complaint and a simple procedure to resolve such a complaint.
